IPDJ sports medical online: what to prepare
For federated athletes: the IPDJ form, when an ECG and vaccination record may be required, and how this differs from a gym certificate (€35).
The IPDJ sports medical is what federations and clubs ask for to register a federated athlete. At Kliniqa the assessment is by video (€45, about 25 minutes). If fitness is confirmed, the doctor completes and signs the official form.
Before the consultation, download the IPDJ form and fill in page 1 (personal details). The doctor completes the clinical section. For under-18s, a parent or guardian should be on the call.
Portuguese sports-medical rules allow the doctor to require a recent resting ECG and a vaccination record with tetanus up to date, especially for competition or if there is cardiovascular risk. Have those ready. If they are missing, the signature can wait until you send them.
This is not the gym certificate. Gyms, fun runs, CrossFit and non-federated university sport use the physical-fitness certificate (€35). If the federation asked for the IPDJ form, book this consultation.
You do not need a family doctor or an SNS number. When issued, the PDF is emailed, signed by a doctor registered with the Portuguese Medical Board.
Frequently asked questions
Will a gym accept this exam?
For non-federated sport the usual document is the physical-fitness certificate, not the IPDJ form. Use the IPDJ exam when the federation or club requires it.
Will I need an ECG?
You might. The doctor decides during the consultation, especially if there is cardiovascular risk or the sport requires it.
Can a foreign athlete in Portugal use this?
Yes. The assessment does not require SNS or a family doctor. The form is for Portuguese federations and clubs.
